Job Classification: Contract Position: Up to 130 hours/month, $40.57 hourly

Position Objective/Summary: The Nursing Consultant will conduct nursing assessments for people with developmental disabilities; conduct needs assessments and develop new health-related programs for children and adults with developmental disabilities; play an essential role in the development of more effective and efficient health care coordination for our clients with developmental disabilities, including those moving out of institutional settings; utilize their clinical skills to assist SCLARC to meet clients’ health needs throughout the continuum of care, including those moving out of institutional settings; actively participate as a member of a multidisciplinary care team to address ongoing needs of SCLARC consumers in multiple environments (hospital, skilled nursing facilities (SNF), intermediate care facilities (ICF) and sub-acute facilities; provide consultation and training for DCSS staff who provide case management services for people with developmental disabilities and their families; provide guidance and support to SCLARC consumers, family members, caregivers, physicians and other health care providers to facilitate the best options to meet consumers’ health care needs including the effective management of health care benefits and costs; be an advocate for consumers to the full extent of their existing health care coverage and programs; and provide oversight/monitoring, consultation, technical assistance and training for community based services providing health care to people with developmental disabilities.
Minimum Qualification Required For This Position: Minimum requirements include an MSN or BSN Degree; must be a Registered Nurse with current CA state license and a minimum of 3-5 years of recent clinical nursing experience; must have a Public Health Nurse Certificate at least 3 years direct experience with persons with developmental disabilities preferred; ability ot exercise independent and sound judgment in decision making, utilizing all relevant information; effective communication skills to facilitate collaboration with members of a healthcare team; good time management and organizational skills along with the ability to manage continually changing priorities; excellent written and verbal communication skills; familiartiy with Microsoft Word and other software programs.
Salary Range: $40.57 per hour
