Position Title: Community Navigator
Title of Immediate Supervisor: FRC Program Manager
Salary Range: $42,287 – $51,400
QUALIFICATIONS:
Candidate must have lived experience as a parent or caregiver of child/adult with developmental/intellectual disabilities or be a self-advocate with experience in advocating for supports and services. Professionals or trusted community leaders with system navigation experience will be considered based on experience. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily including maintaining timelines. An individual must also be proficient in the use of computer applications.
EDUCATION and/or EXPERIENCE:
Associates or Bachelor’s Degree in Social Work, Psychology, Human Services or Child Development or the equivalent of experience (at least five years) in a Human Service agency and experience working with individuals in the field of developmental disabilities.
ADDITIONAL REQUIREMENTS:
Knowledge of the regional center system and services from various generic agencies such as healthcare service systems, insurance supports, and social service agencies. Ability to travel to various work sites and community meetings required. Must be able to work nights and weekends, as needed. Ability to read, write and communicate verbally in Spanish required.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Provide in-person and telephone support to parents of children involved in Early Start and Lanterman services who are at risk for, or diagnosed with a developmental delay or disability. Support may include:
Provide specific guided support to parents of children in the regional center program with accessing programs that will benefit their child’s health and development.
Performs record keeping and data entry for families who are assisted.
Assists families and community agencies with facilitating referrals for regional center service eligibility.
Assist families with appeals processes related to services and supports for the client.
