Title of Immediate Supervisor: Program Manager
Position Objective/Summary: Provides case management services for an assigned caseload. Works closely with consumers and/or their families. Communicates with staff, vendors and the community. Organizes and resolves work-related situations.
Minimum Qualification Required For This Position: Bachelor’s degree (B. A.) from four-year college or university; two years related experience and/or training; or equivalent combination of education and experience. Internships and/or training level employee may have less requirements. Must have two years of early intervention experience 0-3 years of age.
General Statement of Functions: Contacts assigned consumers and documents contacts according to established SCLARC Standard Operating Procedures (SOPs) and policy, and completes required documentation. Prepares case transfers as needed. Attends all unit, division, supervision, staff and in-service training meetings as required. Attends all Planning Team meetings at primary programs and any other consumer related special meetings/hearings (e.g. SCAN, court, IEPs/IFSPs staffing, mental health, governmental/community agencies, placement, etc.) as required. Handles officer-of-the-day (OD) duties as required. Handles confidential information and adheres to Welfare and Institution (W&I) codes. Consults with appropriate specialists regarding the service needs of consumers. Maintains effective working relationships with top management, peers and staff. May act as Program Manager in his/her absence.
Salary Range: $39,718.85 – $58,682.83
