After orientation, the next step is to complete a Person-Centered Plan to determine your goals, dreams, and supports needed. This is a broader, more interactive, process than an Individual Program Plan (IPP), SCLARC requires that you have it completed by someone outside of SCLARC, though your Service Coordinator may attend if you choose for them to – it’s your choice! This will help find unmet needs and changes in circumstances that may impact your budget and address any service disparities. In addition, a Person-Centered Planner can help you with budget and spending plan development and accessing generic resources.
You can choose a provider from our list (providers highlighted in yellow are known to have bilingual staff available), or a community provider of your choice; SCLARC will reimburse up to $2500 per plan after receiving the completed plan and confirming your satisfaction with it. Please note that SCLARC cannot pay for more than one Person-Centered Plan and also may not pay more than $2500.00 for Person-Centered Planning services.
